Admissions to Groton Community Health Care Center

What to Expect

We recognize that an admission to a skilled nursing facility is a new experience for many of our incoming residents, and we are here to lend support to make the admissions process and transition to our facility run as smoothly as possible for all involved. 


Residents can expect a structured environment focused on personalized medical care, daily living assistance, and social engagement. The admission process typically includes an assessment of medical needs, personal preferences, and care requirements. Residents receive assistance with activities of daily living (ADLs) such as bathing, dressing, and medication management.


We provide 24/7 medical supervision, therapy services (physical, occupational, and speech therapy), and coordinated care plans. Meals are provided according to dietary needs, and structured activities, such as group exercises and social events, help promote well-being. Facilities also emphasize safety and comfort, offering private or shared rooms with access to communal spaces.


Family involvement is encouraged, visitation is welcomed. Financial arrangements, including Medicare, Medicaid, and private pay options, should be discussed in advance. Our staff are here to support residents and families throughout the process.

The admission process consists of paperwork to cover important details for both the family and the skilled nursing facility. A variety of sources have regulations that require a resident and/or their representative to be informed of rights, choices to make, procedures to follow, limitations of service, behavioral restrictions, financial obligations, facility policies and other requirements, benefits, and services governing care in the skilled nursing facility.

  • Meal plans, including therapeutic diets, as prescribed by a physician.
  • 24/7 nursing care and assistance with personal care.
  • Use of customarily stocked equipment (e.g. walker, wheelchair). *Excluding any item prescribed by a physician for sole use by a specific resident.
  • Laundry services and bed linens.
  • General household medicine. *Except for specific items as indicated and prescribed for a specific resident.
  • Assistance and/or supervision with ADLs.
  • Activity programs, including religious services.
  • Social services to assist with social, emotional, financial concerns.
